Doctor: Good afternoon. Please, do come in and take a seat.
Patient: Hello. Thank you very much.
Doctor: Could you just give me your name and date of birth, please? And then, tell me how I can help you today?
Patient: My name is Carlos / Carla Koumissi. I was born on the 21st of July, 1970. My skin is giving me a lot of trouble at the moment. It itches a lot. I have to scratch my arms, neck, and even my scalp constantly.
Doctor: I see. Okay, let’s start by getting a bit more information. How are you, generally? Are you eating well? Do you drink alcohol or smoke? And do you exercise regularly?
Patient: Apart from my skin, I’m doing well, thank you. I eat three times a day, and I make sure to have a balanced diet. I do a lot of exercise, I walk everywhere, and I’ve never drunk or smoked. I make sure to stay active.
Doctor: That’s good to hear. It’s great that you’re taking care of your health. When did this problem with your skin start? Has there been any change in your environment recently? Has your skin come into contact with anything new, or does the itching get worse in any particular situation?
Patient: It started about two weeks ago. I don’t think my skin has come into contact with anything new. However, I’ve noticed that when I take a shower or when it’s hot outside, that’s when the itching starts, and I can’t stop scratching. Sometimes I scratch so much that my skin becomes red and sore.
Doctor: Right. Let’s have a closer look then. Could you show me your arms, please? Thank you.
Doctor: From what I can see, it looks like you might be dealing with an allergic reaction, but I suspect this is eczema. It’s quite a common condition that can cause intense itching, especially when the skin is exposed to heat or water. For your arms, I’ll prescribe a cream to apply to the affected area. The good news is that it’s not infected, so we don’t need to worry about any complications there. The most important thing is to keep the skin moisturised and avoid scratching as much as possible. As for your scalp, I recommend using a non-perfumed shampoo, such as Polytar, to help soothe the irritation there.
Patient: Thank you very much. How often should I use the cream and shampoo? Is there a specific time I should apply them?
Doctor: For the cream, you should apply it after washing your arms in lukewarm water. It’s best to do this immediately after showering while your skin is still a bit damp. This way, the cream will help to lock in the moisture. Try to do this at least twice a day. As for the shampoo, I suggest using it once or twice a week. You don’t need to overuse it, as the scalp can get quite sensitive. If you find that it doesn't seem to be helping much, let me know, and we can consider other treatments.
Patient: That sounds good, thank you for the explanation. Do I need to come back for a follow-up, or can I wait before booking another appointment?
Doctor: You don’t need to come back immediately. Only return if the itching persists or if the condition seems to worsen. If the itching continues or if the rash doesn’t improve, we may need to look into further tests. A biopsy of the skin or an allergy test could help us understand whether there is an underlying condition contributing to the symptoms. But for now, we’ll monitor the situation with the cream and shampoo. Here’s your prescription.
Patient: Thank you so much, doctor.
Doctor: You’re very welcome.
